President, Community LIFE
Fatemeh Hashtroudi, MHA, NHA is currently serving as President Community LIFE. Community LIFE is a PACE program serving more than 750 participants in southwestern Pennsylvania. Her prior experience includes Director of Quality Improvement and Patient Safety at Community LIFE for the past 10 years. As part of her responsibilities as the Director of Quality Improvement she implemented LEAN methodology, successfully led Community LIFE’s COVID response as well as implementation of the electronic health record.
Her background includes over twenty years of executive level leadership in long term and acute care operations. Her skills include program planning and implementation, systems design and quality improvement. She has successfully designed and implemented collaboratives with hospitals, hospice and insurance providers. Her background includes extensive experience with regulatory and accreditation agencies including CMS, Joint Commission and CARF-CCAC.
She has a Masters Degree in Health Care Administration from George Washington University and Bachelors of Science in Biology from University of North Carolina at Charlotte. She is Nursing Home Administrator and has training in LEAN: Toyota Production system and Health Care Leadership Development.
Her accomplishment includes design of programs recognized by “Circle of Life”, ACHA –Quality Award, FINE award sponsored by the Jewish Health Care Foundation. She has presented at Leading Age PA and National Hospice Association and National PACE Association on the topics of design and implementation of culture change and hospice and palliative care programming and quality improvement. She serves as Vice-Chair of NPA-Quality Committee. She also has served on Alturum Institute Expert Advisory Committee for PACE Outcome Evaluation.
